Liposana3 - be yourself just better
Non-invasive body contouring has made considerable progress in recent years thanks to multimodal technologies. The Liposana3+ system combines simultaneous high-frequency ultrasound (3 MHz), neuromuscular electrical stimulation (EMS) and photobiomodulation (low-level laser therapy/LED) as well as other frequencies. This simultaneous application is intended to produce synergistic effects that go beyond the sum of individual modalities. In the following, the physiological mechanisms of action, the evidence for synergies and the potential health benefits are scientifically examined.
Technical basics of the Liposana3+ system
The Liposana3+ is a patented device that combines up to six frequency technologies in a single, 30-60 minute reclining treatment. The core components are:
- High-frequency ultrasound (3 MHz) (non-focused): Generates mechanical vibrations and mild thermal effects at defined tissue depths.
- Neuromuscular electrical stimulation (EMS): Induces rhythmic muscle contractions through electrical impulses.
- Photobiomodulation (PBM/LLLT): Red and near-infrared light (600-1000 nm) stimulates cellular processes.
The simultaneous application distinguishes the system from sequential therapies and is intended to produce enhanced metabolic and regenerative effects.
Physiological mechanisms of action of the individual technologies
- Ultrasound (3 MHz) The frequency of 3 MHz leads to a penetration depth of approx. 1-3 cm and generates micromassage and cavitation effects in adipocytes. This makes fat cell membranes permeable, promotes the release of triglycerides and activates lymphatic drainage. Clinical studies show a significant reduction in localized fat deposits. (Studies can be found below)
- Neuromuscular electrical stimulation (EMS) EMS induces supramaximal muscle contractions, which lead to muscle hypertrophy and increased energy consumption. Studies have shown an increase in muscle mass and a secondary reduction in fat through metabolic activation.
- Photobiomodulation (PBM) PBM is primarily absorbed via cytochrome c oxidase in the mitochondria, which increases ATP production, modulates reactive oxygen species (ROS) and activates anti-inflammatory signaling pathways. Meta-analyses confirm increased collagen synthesis and improved microcirculation.
Evidence of synergy with simultaneous application
The simultaneous combination of several energies demonstrably leads to over-additive effects:
- Ultrasound temporarily opens the adipocyte membranes → EMS "pumps" released lipids out of the tissue through intensive muscle work → PBM simultaneously increases cellular energy (ATP), reduces oxidative stress and promotes new collagen formation for skin tightening.
- Studies of comparable combination therapies (e.g. ultrasound + electrical stimulation or PBM + other modalities) show significantly better results for body contouring than monotherapies. A pilot study with non-focused ultrasound + electrotherapy reported improved cellulite, skin firmness and fat reduction (see appendix for the names of the studies)
- Synergies between PBM and mechanothermal stimulation and PBM result in increased lipolysis, muscle tone and tissue regeneration.
Although specific randomized controlled trials on the Liposana3+ system itself are currently lacking, the physiological mechanisms and analog studies on multimodal devices support the hypothesis of superior efficacy through simultaneous application.

Mechanisms of action and observed effects of the combined technologies described in the scientific literature
The following effects are reported in studies on the individual technologies (high-frequency ultrasound, neuromuscular electrical stimulation and photobiomodulation) and on comparable multimodal procedures or are declared to be physiologically plausible:
- Fat reduction & body contouring
Clinical studies on high-frequency ultrasound and combined approaches document a measurable reduction in circumference. This is attributed to increased lipolysis and activation of lymphatic drainage[1,3]. - Muscle building & toning
Neuromuscular electrical stimulation (EMS) leads to an increase in muscle mass and higher muscle tone in controlled studies. In multimodal protocols, an improvement in muscle tone and postural stability is also observed[2]. - Skin tightening & cellulite reduction
Photobiomodulation (PBM) has been shown to stimulate fibroblast activity and the formation of new collagen and elastin. Several clinical studies associate these mechanisms with an improvement in skin firmness and a visual reduction in the appearance of cellulite[1,4]. - Anti-inflammation & pain modulation
Photobiomodulation shows a reduction in inflammatory markers (e.g. TNF-α, IL-6) and a modulation of pain mediators in meta-analyses. Combined procedures are therefore associated with the alleviation of musculoskeletal complaints in the literature[1,5]. - Metabolic & systemic effects
By stimulating the mitochondrial respiratory chain (increased ATP production) and improving microcirculation, studies have described increased cellular energy availability and potential immunomodulatory effects[1,2].
The above observations are based on the physiological principles of action of the individual technologies and on the results of comparable multimodal systems. Specific randomized studies on the Liposana3+ system itself are currently (as of November 2025) still available to a limited extent.
Conclusion
The simultaneous combination of ultrasound, neuromuscular stimulation and photobiomodulation in the Liposana3+ system utilizes well-established physiological mechanisms to achieve synergistic effects in non-invasive body contouring and tissue health. While the evidence on the individual technologies is robust and analog studies confirm multimodal synergies, prospective RCTs on the overall system would be desirable to clearly quantify the superiority.

Supplementary evidence-based studies on the mechanisms of action of the technologies in multimodal systems such as Liposana3+
Further relevant clinical studies and meta-analyses are listed below (as of November 2025) to provide more in-depth information on the physiological effects of the core technologies (high-frequency ultrasound, neuromuscular electrical stimulation/EMS and photobiomodulation/PBM) and comparable multimodal approaches. The selection is based on randomized controlled trials (RCTs) and systematic reviews with high methodological quality.
1. high-frequency ultrasound (3 MHz range or HIFU) - fat reduction & body contouring
Clinical studies document a circumference reduction of 2-5 cm after a few sessions, primarily through lipolysis and lymph activation:
- Jewell et al. (2011): RCT with HIFU; average circumference reduction >4 cm on the abdomen after one session.
- Fatemi (2009): Average reduction of 4-5 cm waist circumference; histologically confirmed adipocyte disruption without damage to the dermis.
- Teodoro et al (2018): Pilot study on non-focused ultrasound + current therapy; significant improvement in cellulite and fat reduction through synergy.
2. neuromuscular electrical stimulation (EMS) - muscle building & toning
RCTs show increases in muscle mass of 10-20% and secondary fat reduction through metabolic activation:
- Kemmler et al. (2016-2023): Several RCTs on whole-body EMS; comparable to high-intensity strength training: +1-2 kg muscle mass, -1-3 kg fat mass after 12-16 weeks.
- Kim & Lee (2018): RCT in abdominal obesity; significant reduction in waist circumference and increase in muscle mass vs. sham control.
- Vranis et al. (2024): Combination EMS + radiofrequency; average 19.6 % subcutaneous fat reduction and muscle hypertrophy.
3. photobiomodulation (PBM/LLLT, red and near-infrared light) - skin tightening, cellulite & anti-inflammation
Meta-analyses and RCTs confirm stimulation of fibroblasts, new collagen formation and reduction of inflammatory markers:
- Hamblin (2017): Comprehensive review; PBM reduces pro-inflammatory cytokines (TNF-α, IL-6, IL-1β) and modulates NF-κB signaling pathway.
- Lachérrade et al. (2013): Double-blind RCT (n=68); significant improvement in the appearance of cellulite on thighs with 532-nm LLLT alone.
- Avci et al (2013) & Hamblin (2016): PBM increases ATP production, collagen synthesis and microcirculation; clinically associated with firmer skin and reduced cellulite.
4. multimodal combinations (ultrasound + EMS + PBM/RF or comparable)
Studies on simultaneous or sequential combinations show over-additive effects:
- Theodorou et al. (2024): Prospective multicenter study (RF + EMS); significant reduction in subcutaneous fat layer thickness (3.1 mm) and waist circumference (1.9 cm) after only 3 sessions.
- Chang et al (2013 & 2016): Focused ultrasound + radiofrequency in Asian patients; effective circumference reduction with high satisfaction and without relevant side effects.
- Alizadeh et al. (2024): Narrative review of multimodal devices (incl. HIFU + RF + EMS); superior results over monotherapies for fat reduction, muscle tone and skin tightening.
Summary of the evidence base
The individual technologies are well documented by numerous RCTs and meta-analyses. Multimodal simultaneous applications (as in Liposana3+) clearly show synergistic effects in analog studies - faster and stronger lipolysis, muscle hypertrophy and regenerative processes through mutual reinforcement of cellular mechanisms. Specific RCTs on the exact Liposana3+ protocol are still lacking, but the physiological principles and results of comparable systems support the hypothesis of superior efficacy.
